ABSTRACT

Presentare con logica multiprofessionale, utilizzando come indicatore di efficacia assistenziale il contenimento delle infezioni nosocomiali in area critica, le modalità di gestione di alcune problematiche legate al bisogno di:
b) eliminazione urinaria
Quando la decisionalità è non solo professionale, ma anche deontologica nelle scelte assistenziali per la prevenzione e contenimento delle infezioni ospedaliere

ABSTRACT
Le Infezioni Ospedaliere sono tra i principali indicatori di qualità dell’assistenza e la cateterizzazione vescicale rappresenta uno dei più alti rischi di contrarre infezioni del tratto urinario.
Linee Guida nazionali del Ministero della Sanità ed anche quelle internazionali affermano che la prima misura di prevenzione della infezioni delle vie urinarie è, ridurre l’esposizione del paziente al cateterismo vescicale.
Si è voluto esplorare il bisogno dell’eliminazione urinaria, avendo come indicatore di qualità la prevenzione e contenimento delle infezioni ospedaliere da cateterismo vescicale, identificando quegli interventi assistenziali che rispondono all’EBN/M (Evidence Based Nursing/Medicine) all’appropriatezza, ed alla razionalizzazione delle risorse.
Il cateterismo vescicale diventa risposta ad un bisogno, quando è prestazione appropriata.
Dovrebbe quindi, essere utilizzato in determinate situazioni cliniche, in presenza di EBM e andrebbe rimosso, non appena viene a mancare la necessità.
La domanda è: che cosa va considerato “necessario” o meglio essenziale per il paziente?
La centralità del paziente è assicurata dalla forte considerazione degli outcomes attesi, dall’adozione di protocolli e procedure validati scientificamente nell’applicazione del catetere vescicale e da una corretta gestione del paziente cateterizzato, nonché da una adeguata gestione del malato con problemi di eliminazione, ma non necessariamente cateterizzato; in questi casi, si pone il problema della valutazione dei sistemi alternativi. A questo livello la partecipazione dell’infermiere nelle scelte e nella gestione assistenziale deve essere molto attiva ed estremamente valorizzante.
Decisionalità non solo professionale, ma anche deontologica quando si tratta di indicazioni a scelte di presidi e materiali, e quando si tratta di dover eradicare convinzioni inveterate e abitudini assistenziali non corrette.
